Our referral process ensures that individuals receive the most appropriate support to meet their needs. Here's how it works:
1. Initial Referral: Referrals can be made by professionals, individuals, or family members using our online form, by email, or by phone.
2. Assessment: Our team will conduct a comprehensive assessment to understand the individual's needs, preferences, and goals.
3. Support Planning: Based on the assessment, we develop a personalized support plan that outlines how we'll meet the individual's needs.
4. Service Agreement: A formal agreement is established between IHcPro, the individual, and where relevant, the funding authority.
5. Service Commencement: Support services begin according to the agreed schedule, with regular reviews to ensure ongoing suitability. Throughout this process, we maintain clear communication with all parties involved to ensure transparency and collaboration.

who can make a referral

we accept referral from a range of sources to ensure individuals can access our services through multiple pathways

Local Authorities

Social workers and care managers from any of the Permitted Purchasing Bodies within the Liverpool City Region, including Knowsley, Halton, Liverpool, Sefton, St Helen's, and Wirral Councils.

Health Professionals

Healthcare professionals from NHS services, including Clinical Commissioning Groups, community mental health teams, and hospital discharge teams.

Self-Referrals

Individuals seeking support services can refer themselves directly, including those with eligible needs who have a personal budget or direct payment.

Family Members

Family members or carers can make referrals on behalf of an individual who may require support services.

Educational Institutions

Special Educational Needs Coordinators (SENCOS) and transition teams supporting young people preparing for adulthood.

Advocacy Services

Independent advocates working with individuals to ensure their voices are heard in decisions about their care.

Eligibility Criteria

Our non-CQC registered community-based support services are available to adults (18+) and young people preparing for adulthood who have eligible needs under the Care Act 2014. We provide services to individuals with a range of needs, including:

Learning disabilities
Physical disabilities
Mental health needs
Sensory impairments
Autism spectrum conditions
Complex needs requiring specialized support

Additionally, individuals should:

Have an identified funding source (e.g., local authority funding, direct payment, personal health budget) or be self-funding
